Impact of COVID-19 lockdown on sleep quality of pharmacy students in UiTM Puncak Alam

Due to the COVID-19 pandemic, the closure of educational institutions was executed during the period of lockdown. This subsequently led to alterations in daily routines and emotional distress, particularly among university students, affecting their sleep quality resulting in disturbance of immune functioning and mood regulation. Hence, the present study aimed to measure students' sleep quality during the first few months of the lockdown period. This study is a cross-sectional, single-centred survey that was done by distributed electronic questionnaire. The e-questionnaire was divided into 4 sections that assessed socio-demographic characteristics, sleep quality, psychological state and engagement in physical activity. Exactly 248 valid responses were received, 82 % female students. Students with poor sleep quality were three times greater than those with good sleep quality. No association were found between physical activity and sleep quality. However, a significant positive relationship between sleep quality and psychological state was observed (p < 0.01). In conclusion, this study suggests that the majority of pharmacy students in UiTM Puncak Alam are poor sleepers and psychological distress negatively affects sleep quality but, is mainly due to stress. Thus, interventions such as stress management programs and mental health support can be taken by the faculty's management to overcome this problem.     

Patient-Centered Framework for Rehabilitation Research in Outpatient Settings

Conducting high-quality clinical research is dependent on merging scientific rigor with the clinical environment. This is often a complex endeavor that may include numerous barriers and competing interests. Overcoming these challenges and successfully integrating clinical research programs into clinical practice settings serving rehabilitation outpatients is beneficial from both a logistical perspective (eg, supports efficient and successful research procedures) and the establishment of a truly patient-centered research approach. Leveraging our experience with navigating this research-clinical care relationship, this article (1) proposes the Patient-Centered Framework for Rehabilitation Research, a model for integrating patient-centered research in an outpatient clinical setting that incorporates a collaborative, team-based model encompassing patient-centered values, as well as strategies for recruitment and retention, with a focus on populations living with disabilities or chronic diseases; (2) describes application of this framework in a comprehensive specialty multiple sclerosis center with both general strategies and specific examples to guide adaptation and implementation in other settings; and (3) discusses the effect of the framework as a model in 1 center, as well as the need for additional investigation and adaptation for other populations. The 5 interconnected principles incorporated in the Framework and which prioritize patient-centeredness include identifying shared values, partnering with the clinical setting, engaging with the population, building relationships with individuals, and designing accessible procedures. The Patient-Centered Framework for Rehabilitation Research is a model presented as an adaptable roadmap to guide researchers in hopes of not only improving individual patients’ experiences but also the quality and relevance of rehabilitation research as a whole. Future investigation is needed to test the Framework in other settings.     

应用锥形束CT对种植区牙槽骨质量分类方法的改良研究

目的    应用锥形束CT（cone beam CT，CBCT）测量种植区牙槽骨质量并结合Lekholm和Zarb分类法对骨质量分类方法进行改良，以期为提高牙槽骨质量分类的准确性提供指导。方法    选择2015年10月至2017年10月在呼和浩特市口腔医院种植科进行种植修复的196例患者术前CBCT影像资料，应用 Invivo5诊断设计软件测量306个种植位点的牙槽骨皮质骨厚度和松质骨密度（皮质骨与松质骨的CT值差值），以Lekholm和Zarb分类法为基础结合测量数据对骨质量进行改良后的量化分类。结果    牙槽骨皮质骨厚度为0.18 ~ 2.89 mm，中位数为0.92 mm。皮质骨与松质骨的CT值差值为88.5 ~ 667.8 HU，应用百分位数法找到33.3%和66.6%对应的数值分别为297.8 HU和356.1 HU。依据此数据结合Lekholm和Zarb分类法将196例患者的306个种植位点骨质量分为4类，其中Ⅰ类骨种植位点46个（占15.0%），Ⅱ类骨104个（占34.0%），Ⅲ类骨114个（占37.3%），Ⅳ类骨42个（占13.7%）。结论    结合Lekholm和Zarb分类法，应用CBCT测量种植区牙槽骨质量并进行分类方法的改良，可为临床医生术前评估牙槽骨质量提供一定参考。     

Collaborating toward equity in Pennsylvania: The Age-Friendly Care,PA project

Objective

To collaboratively implement the age-friendly health systems framework, known as the 4Ms: What Matters, Medication, Mentation, and Mobility, at The Primary Health Network (PHN), a federally qualified health center.

Data Sources

Data were collected from PHN electronic medical records (EMRs) for individuals over age 65 from December 30, 2019 to December 24, 2021 and from Project ECHO© attendance and evaluation surveys.

Study Design

The telementoring educational program, Project ECHO©, was used to engage PHN health care professionals working in rural areas of Pennsylvania to incorporate the 4Ms into their practice starting with the annual wellness visit (AWV). Project ECHO© was launched at three primary care sites. After 18 months, it was then disseminated to an additional 18 sites creating pilot and comparison groups. Outcomes included codesigned patient process metrics using EMR data and project ECHO© participant data.

Data Collection Methods

EMR data were generated by system reports created by PHN's quality assurance program manager. Project ECHO© data were collected and managed using REDCap electronic data capture tools. Outcomes were aggregated, analyzed for trends over time, and compared between groups.

Principal Findings

All nine process outcomes increased from baseline to follow-up at the three initial sites, ranging from 4% to 43% g. At year two, the three initial sites had higher rates on AWVs (pilot 24%, comparison 12%; p < 0.0001), Advance Care Planning (New on file, pilot 8%, comparison 2%; Discussed with patient, pilot 18%, comparison 13%; Patient declined, pilot 0%, comparison 0%; p = 0.0001), Dementia Screening (pilot 24%, comparison 12%; p < 0.0001), Fall Risk Management (pilot 43%, comparison 10%; p < 0.0001), and Mobility Goal (pilot 19%, comparison 9%; p < 0.0001); and lower rates on High-Risk Medication Elimination (pilot 54%, comparison, 63%, p < 0.02).

Conclusions

Access to high-quality geriatric care for rural older adults can be improved by increasing health care professionals' knowledge of the 4Ms, beginning with its incorporation into the AWV.     

园艺互动干预应用于维持性血液透析病人中的效果评价

目的:探讨园艺互动干预应用于维持性血液透析病人中的效果。方法:选择本院收治的行维持性血液透析病人157例,按照入院时间先后顺序分为对照组77例,观察组80例。对照组实施常规干预,观察组实施园艺互动干预。对比两组干预前后心理状态、应对方式、生活质量及总体幸福感变化。结果:观察组干预12周后症状自评量表(SCL-90)各维度评分低于对照组(P<0.05);观察组干预12周后积极应对评分高于对照组,消极应对评分低于对照组(P<0.05);观察组生活质量各维度评分高于对照组(P<0.05);观察组总体幸福感评分高于对照组(P<0.05)。结论:园艺互动干预应用于维持性血液透析病人可改善心理状态及应对方式,提升生活质量与总体幸福感。

新疆低温奶消费情况调研报告

本文旨在探讨影响新疆低温乳消费市场的限制性因素及相应的对策。调研通过网络调查问卷和实地调研走访相结合的方式,从乳制品消费结构、消费能力,低温奶认知、低温奶购买意愿、购买便利性、购买偏好及健康饮奶习惯等方面分析影响新疆低温乳消费市场的因素。结果显示:32.93%的消费者不了解乳中生物活性物质(如乳铁蛋白)的热敏性,39.97%的消费者不能区分各类液态乳产品,47.64%的消费者有煮沸巴氏杀菌乳后饮用的习惯;43.21%的消费者"担心未消毒彻底,存在细菌",36.30%的消费者认为"加热后可增加营养价值和香味";72.64%的消费者曾感到"购买巴氏杀菌乳不方便";54.85%的消费者认为本地品牌"更新鲜、更健康"。结果启示:本地乳企应该充分利用在当地的良好品牌信誉度和牧场资源,尽早布局低温奶市场,通过研发高品质低温乳制品、普及科学饮奶理念、改善冷链物流能力、完善购物体验和精细化服务能力等手段,提高生产竞争力,抵御进口乳制品和常温乳对当地乳品市场的冲击,占领区域低温奶消费市场。     

A cut above the rest: Trimming as a painless depilatory

Shaving and other modes of epilation can cause undue anxiety, pain, or skin irritation in children. Here, we present hair trimming as a safe, painless, and cost‐effective alternative for patients with unwanted hair which may be performed indefinitely or until the child is old enough to direct management. In select cases, removing unwanted hair using this technique may facilitate dermatologic surveillance.
